I think it was Rad Racer beating Mad Arab for the World Title, the worst about it was that it was in a match in the March Madness Tournament, and i just booked this match at the last second, that match was supposed to be just another easy title defense for Mad Arab, but it ended this way

Freedom fighter and Juan Tijuana had my tag titles for like a year.
Jobber Joe was cruiserweight champ for a few months, and I think even won the TV title.
Lifeguard chip had my world title for about a month.
Ladies title, not really many weird champions. The weirdest was probably the shmuck.

I threw mike bugg in a triple threat for the cruiserweight championship he exactly won it. He had a nice run with it to defending the belt for a few months

At March Madness 1990 I had all my best wrestlers in the tournament so gave Dude Brohan a surprise opponent for his World Title defence. Having successfully defended against Lazer, Buzzsaw and JSS in past matches he lost the title to Valkyrie!

This isn't a championship, but jobber Joe won my cw cup. He did go on to win the title tho. I always make tournament winners get title shots.

My all-time wierdest:
CW: Devin Van Winkle held the title for years and was undefeated in non-title matches as well, then suddenly lost to Cletus Mullet
Others:
World: Flex Deltoid beat 90s legendary Mercenary when multiple main eventers couln't pull it off
TV: Nothing too crazy, biggest surprise was probably Jessie Wild beating O'Hooligan for it while Chet Skye was out injured.
Women: Honestly I've hardly had anyone able to keep the title past one defense other than Trixie and Femme Fatale. Didn't have any main eventers in my last 80s run though so may play out differently since I'm about to roll over past 1999 again and have a much deeper women's roster to start with.
Side Note: Still waiting to hear about a Local Talent winning a belt.

He won the world title. I only put in the match because I was in the beginning of my fed and had one to really compete. He lost title the next match.

I had the same problem with the ladies title until I got valkary. She held the title for 2 or 3 years.

Still weird tho, considering its dude brohan. What's his push?

It may not be weird, but it was sure interesting. I was bored in my run, and I was a big fan of Pelvis Pressly, so I made him win the World Title, the TV title, the Cruiserweight championship and the Tag Team championships (with Flex Deltoid) All at the same time. Yep. He defended most of them for a year, and n Beyond War Dome, he lost the World Title to Arizona Chance, the tag titles to 8-Bit, and I think he was left with the TV and the CW titles. He didn't last long with those, tho. I love that story, Pelvis became my top guy.

He lost it in the next week though.
